Aquabot Repair Form

If you are sending us your Aquabot for repair, please complete the following form. When you submit the form, one copy will be sent to us (so we know your repair is coming)  and one copy will be sent to your email address.

Print out your copy and enclose it when you ship the unit to us.

If your Aquabot is being sent to us for something other than simple parts replacement, it is often a good idea to send us your power supply so that we can test it as well.

Please print a copy of this form and include it in the box with the product you are sending to us.
